how to translate "Form"?

Hi, I am translating GoldenDict into slovak language and I am not sure how to translate the string "Form" -it has two different meanings: 1,shape/condition of something 2,somtehing to fill out. I was not able to find where exactly is this string located in GoldenDict, therefore I can't decide which meaning it was meant to have. Even looking in other translations didn't help me. So please can anybody help me and tell what is the meaning of this word in this case? Thanks and sorry for my poor English.
P.S.: It would be nice if this site had favicon :)

Just checked all occurrences of the word so I'm pretty sure that means "something to fill out"

Hmm. The fact that I've auto-corrected 'Form' to be 'From' is by itself an indicative that there's no such word in use in the program. Most probably it's a boilerplate text added when some of the forms were created in Qt Creator. The right fix is to just mark it as non-translatable in the source. Meanwhile, you can just translate in as 'Form'.
